- Left thumb impression: black or blue ink on white paper (200 DPI, 3 × 3 cm).
- Handwritten declaration: in English, in your own handwriting, black ink on white paper, not in capital letters (200 DPI, 10 × 5 cm).
- Declaration text: "I, ____ (Name of the candidate), hereby declare that all the information submitted by me in the application form is correct, true and valid. I will present the supporting documents as and when required."
